Support in the daily life of people living with mental health problems to support their social retention in their community.* Home visits focusing on daily living skills, coping skills for mental health and the development of natural support in the community.* Support groups.* Telephone contacts.* Reference to appropriate community resources.* Assistance in finding housing in the West Island.
