Services

Palliative care centre.

* Possibility for relatives to live with the resident.

* Free services to the residents: food, medical care and hygiene, nursing services, psychosocial support, aesthetic care (hairdressing, manicure-pedicure), osteopathy, massage therapy and spiritual and family support.

* Bereavement workshops.

* Bereavement support groups.

* Weekly bereavement coffee meetings.

* Individual bereavement counselling.

on referral, contact the Guichet d'accès en soins palliatifs de Lanaudière (CISSS de Lanaudière) by phone at 450 471-2881 ext. 4055, requests from the medical community: by fax at 450 654-0262, by e-mail at guichetsadsud.cissslan@ssss.gouv.qc.ca all documents required for admission

Documents required

application for admission, pre-admission consent signed by the user or their representative, medical note proving prognosis less than 3 months old, latest relevant nursing and medical notes, up-to-date medication profile, level of care D and DNR order, user's contact information, imaging or examination report demonstrating the evolution of the disease
