make a request: fill in a form on the website, obtain information on the service: make an appointment by phone or email

Documents required

for all members of the household: birth certificates if born in Canada or citizenship or permanent residence cards, obligatory according to the situation: original lease and most recent renewal notice, tax assessment for all adults over 18 years old, alimony received, bank receipts for the last 2 months for all adults over 18 years old, proof of school attendance for students over 18 years old who attend an educational institution full time, child custody decree
